Sageli on patsiendiks \u00f5lav\u00f6\u00f6tmepingetega kontorit\u00f6\u00f6taja v\u00f5i vastupidi f\u00fc\u00fcsiliselt n\u00f5udliku t\u00f6\u00f6 v\u00f5i hobiga inimene. Kuigi kaela- ja \u00f5lav\u00f6\u00f6tmevalu v\u00f5ib kirjeldada eraldi, on probleemid tihti omavahel tihedalt seotud.<\/p>\n\n\n\n<p>L\u00fclisamba kaelaosa moodustavad seitse kaelal\u00fcli, mille \u00fclesanne on toestada pead ning v\u00f5imaldada selle liikumist. Kaelal\u00fclidele lisaks on l\u00fclisambas liigesed, sidemed, vahekettad ehk diskid, n\u00e4rvid ja seljaaju. Kaelaosale on loomulik lordoos ehk ettepoole kumerus. \u00d5lav\u00f6\u00f6tme moodustavad rangluu ja abaluu ning nendele kinnituvad lihased, mille \u00fclesanne on \u00f5lavarre liigutamine ette, taha, k\u00f5rvale, p\u00f6\u00f6rata seda nii sisse kui v\u00e4lja, tuua \u00f5lavart tagasi alla. K\u00f5rvalekalle v\u00f5i vigastus m\u00f5nes eelnimetatud kaela-\u00f5lav\u00f6\u00f6tme struktuuris v\u00f5ib p\u00f5hjustada valu v\u00f5i muid ebamugavaid kaebusi. Nendeks v\u00f5ivad olla:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>j\u00e4ikus (liikuvuspiiratust) l\u00fclisamba kaelaosas, ka \u00f5lav\u00f6\u00f6tmes;<\/li>\n\n\n\n<li>pingepeavalud;<\/li>\n\n\n\n<li>lihasspasmid v\u00f5i valupunktid lihastes;<\/li>\n\n\n\n<li>tuimustunne, n\u00f5rkus ja surinad ehk paresteesiaid \u00fclaj\u00e4semetes;<\/li>\n\n\n\n<li>kiirguv valu;<\/li>\n<\/ul>\n\n\n\n<p>Kaelavalu v\u00f5ib jagada kaheks: aksiaalseks ehk kaela- ja \u00f5lav\u00f6\u00f6tme piiresse j\u00e4\u00e4v ning radikulaarne ehk kiirguv valu, mis kandub edasi \u00fclaj\u00e4semesse. Kiirguva valu korral v\u00f5ib olla tegemist l\u00fclisamba kaelaosa diski v\u00e4ljasopistumisega, mis surub n\u00e4rvile ning \u00e4rritab seda (IQWiG, 2010).<\/p>\n\n\n\n<p>V\u00e4ga harva on kaelavalu m\u00e4rk t\u00f5sisemast probleemist. Ohuteguriteks, mille puhul tuleb kindlasti kiiremas korras arstiga \u00fchendust v\u00f5tta, on (Nordin <em>et al., <\/em>2009):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>trauma j\u00e4rel tekkinud kaelavalu;<\/li>\n\n\n\n<li>kaela-\/kuklakangestus;<\/li>\n\n\n\n<li>p\u00f5ie-v\u00f5i sooletegevuse kontrolli kaotamine;<\/li>\n\n\n\n<li>enneolematu peavalu iivelduse, oksendamise, pearingluse v\u00f5i valgustundlikkusega;<\/li>\n\n\n\n<li>peavalu, mis p\u00fcsib muutumatu olenemata asendist;<\/li>\n\n\n\n<li>seletamatu kaalulangus, k\u00fclmav\u00e4rinad, palavik;<\/li>\n\n\n\n<li>tundlikkuseh\u00e4ired: v\u00f5imetus liigutada \u00fclaj\u00e4set, s\u00f5rmi vms.<\/li>\n<\/ul>\n\n\n\n<p>\u00dchtlasi jaguneb valu nii aktuuseks ehk \u00fchekordse kuluga probleemiks kui ka krooniliseks ehk kauemat aega kestvaks. Akuutne valu kestab tavaliselt alla kolme kuu, krooniline \u00fcle kolme kuu (Misailidou <em>et al<\/em>., 2010).<\/p>\n\n\n\n<p>M\u00f5ned enamlevinud kaela-\u00f5lav\u00f6\u00f6tme valu p\u00f5hjustajad on:<\/p>\n\n\n\n<ol class=\"wp-block-list\" type=\"1\">\n<li><strong>N\u00f5rgad v\u00f5i \u00fclekoormatud kaela- v\u00f5i \u00f5lav\u00f6\u00f6tmelihased<\/strong><\/li>\n<\/ol>\n\n\n\n<p>&nbsp;Probleem tuleb eriti ilmsiks n\u00e4iteks kontorit\u00f6\u00f6tajate, juuksurite ja hambaarstide seas, kes p\u00fcsivad tunde sundasendis, kus kaela-\u00f5lav\u00f6\u00f6tmelihased on pidevas isomeetrilises kontraktsioonis ehk lihases on pidev pinge.&nbsp; Pinges lihas pole aga tugev lihas. N\u00f5rkade, kuid pidevalt pinges lihaste korral tuleb teha pidevaid pause sundasenditest ning suurendada kaela-\u00f5lav\u00f6\u00f6tmelihaste j\u00f5udu ja vastupidavust edaspidi toimetulekuks (Saeterbakken <em>et al.<\/em>, 2012).<\/p>\n\n\n\n<p>&nbsp;Lisaks v\u00f5ib lihaspinge esile kutsuda pingepeavalusid, kus on tunda survet tihedalt pea \u00fcmber ning tavaliselt kahepoolselt. Valu avaldub p\u00e4eva l\u00f5pupoole, n\u00e4itamaks m\u00f5jutust stressist ja koormusest. Pingepeavalu allub h\u00e4sti ravile k\u00e4sim\u00fc\u00fcgiravimite ja l\u00f5dvestusharjutustele ning massaa\u017eile (Chowdhury <em>et al. <\/em>2012).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Liigeskulumus ehk osteoartroos<\/strong><\/li>\n<\/ul>\n\n\n\n<p>On loomulik, et vanemaks saades esineb liigestes kulumist. Diskid lamenevad ning ainevahetush\u00e4irete t\u00f5ttu v\u00f5ivad tekkida l\u00fclisambasse osteof\u00fc\u00fcdid ehk luukasvised. Selline j\u00e4igastumine muudab kaela liigutamise raskemaks. On t\u00f5en\u00e4oline, et r\u00f6ntgen v\u00f5i MRT-pilt n\u00e4itab kulumist, samas valu kaelas ei esine. Eakohased muutused l\u00fclisambas on loomulikud. Vaatamata sellele v\u00f5iks liigesliikuvust ja ainevahetust parandada liikuvus- ja j\u00f5uharjutustega kaela- ja \u00f5lav\u00f6\u00f6tmepiirkonnale (Novick <em>et al.<\/em>, 2018).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><em>Whiplash<\/em><\/strong><strong> ehk piitsal\u00f6\u00f6gi t\u00fc\u00fcpi vigastus<\/strong><\/li>\n<\/ul>\n\n\n\n<p><em>Whiplash<\/em>-t\u00fc\u00fcpi vigastusi kutsuvad esile \u00e4kilised kiirendus-aeglustus liigutused, mis venitab kaelalihaseid ja -k\u00f5\u00f5luseid. Kaasneda v\u00f5ivad kaelavalu, mittespetsiifiline peavalu, pearinglus, tendomandibulaarliigese ehk alal\u00f5ualuuliigese valulikkus. Oht on valu krooniliseks muutumiseks (Kazeminasab <em>et al., <\/em>2022). Oluline on puhkuse j\u00e4rel ja valuravi k\u00f5rval v\u00f5imalikult ruttu naasta \u00f5nnetuse-eelsete tegevuste juurde ning sooritada spetsiifilisi harjutusi v\u00f5i f\u00fcsioterapeudi juures mobiliseerivaid harjutusi (McClune <em>et al., <\/em>2002).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>L\u00fclisambakanali ahenemine v\u00f5i diski v\u00e4ljasopistumine<\/strong><\/li>\n<\/ul>\n\n\n\n<p>Kui l\u00fclisambakanal on liiga kitsas v\u00f5i tekib diski v\u00e4ljasopistumine, v\u00f5ib n\u00e4rvijuurele avalduda surve, mis kiirgub edasi valuna v\u00f5i \u201csipelgate jooksmisena\u201d kaelast \u00fclej\u00e4semesse. Alati ei tekita diski v\u00e4ljasopistumine s\u00fcmptomeid ning diskid v\u00f5ivad paraneda inimesele teadmata m\u00f5ne kuu jooksul. Kergete kuni m\u00f5\u00f5dukate s\u00fcmptomite esinemisel suunab arst patsiendi f\u00fcsioteraapiasse, kus f\u00fcsioterapeut hindab probleemi ning paneb selle p\u00f5hjal harjutuskava kokku, aitamaks kaasa diski paranemisele ja valu v\u00e4hendamisele (Matsumoto <em>et al. <\/em>2001).<\/p>\n\n\n\n<p><strong>Diagnoosimiseks<\/strong> kasutatakse lisaks patsiendi k\u00fcsitlemisele piltdiagnostikat, millele saab toetuda kattuvate s\u00fcmptomite esinemisel. Lisaks hindab f\u00fcsioterapeut teraapias l\u00fclisamba kaelaosa ja \u00f5lav\u00f6\u00f6tme liikuvust ja j\u00f5udu ning sooritab vajadusel spetsiifilisi teste (nt Spurlingi test), m\u00f5istmaks n\u00e4rvide v\u00f5i l\u00fclisamba seisundit t\u00e4psemalt.<\/p>\n\n\n\n<p>Vahel ei saa kaela- ja \u00f5lav\u00f6\u00f6tmevalu \u00fchegi kindla kategooria alla liigitada, sest erinevad koed (luud, liigesed, lihased jne) on l\u00e4hestikku koos. Eriti keeruline on valuallika leidmine kroonilise valu puhul. Sel juhul nimetatakse seda <strong>mittespetsiifliseks<\/strong>. Kroonilise mittespetsiifilise valu puhul v\u00f5ivad stress, depressioon, \u00e4revus ja uneprobleemid m\u00f5jutada selle ulatust ning esinemissagedust (Kazeminasab <em>et al., <\/em>2022). Lisaks stressi maandamisele, \u00e4revuse ja depressiooni kontrolli alla saamisele ja unerutiini parandamisele on abiks nii j\u00f5uharjutuste sooritamine kaela- ja \u00f5lav\u00f6\u00f6tmelihastele n\u00e4iteks 3 korda n\u00e4dalas 30 minuti jooksul (Ylinen <em>et al<\/em>., 2003).<\/p>\n\n\n\n<p>Kaelavalu <strong>ennetamise <\/strong>puhul kehtivad sarnased p\u00f5him\u00f5tted ravile: oluline on v\u00e4ltida sundasendeid, leida l\u00f5\u00f5gastustehnikad stressi maandamiseks ja \u00e4revusega toimetulekuks (<em>mindfulness<\/em>, l\u00f5\u00f5gastav massaa\u017e), hoida end pidevas liikumises ning sooritada passiivse eluviisi korral j\u00f5uharjutusi kaela- ja \u00f5lav\u00f6\u00f6tmelihastele.<\/p>\n\n\n\n<p><strong>Kasutatud kirjandus<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Chowdhury D.
